MEMBER COMMUNICATION POLICY

By accepting this Policy, you hereby consent and authorize healthio inc., or its affiliates and/or authorized business associates (“collectively, “healthio”), to communicate with you through the various methods described below.  You acknowledge your recognition and understanding of the inherent risks of communication through electronic methods such as unsecured or unencrypted email and/or text.  You understand that these communications may involve the use and/or disclosure of your protected health information (“PHI”) to further treatment, payment or healthcare operations or as otherwise authorized by you (“TPO”).  Finally, you understand that you can change your communication preferences at any time by contacting healthio via the information provided in the Privacy Policy.

 

Telephone

With this consent and authorization, you agree that healthio my contact you via your home number, mobile number or any other number you provide us and either speak with you directly or leave a message on voicemail regarding any topic related to the TPO, such as troubleshooting issues related to the healthio app or devices, reminders to measure your health vitals and perform other scheduled and unscheduled tasks as determined by your insurance plan, employer or your medical team, and check-in calls to assist you in meeting your health goals, among other health related topics. 

 

E-mail

With this consent and authorization, you agree that healthio may contact you via the unsecured or unencrypted email messages regarding any topic related to the TPO, such as information to troubleshoot issues related to the healthio app or devices, reminders to measure your health vitals and perform other scheduled and unscheduled tasks as determined by your insurance plan, employer or your medical team, tips and tricks to assist you in meeting your health goals, and general health and wellness information, among other health related topics.

With this consent and authorization, you agree that healthio may contact you via the unsecured or unencrypted SMS text messages regarding any topic related to the TPO, such as information to troubleshoot issues related to the healthio app or devices, and reminders to measure your health vitals and perform other scheduled and unscheduled tasks as determined by your insurance plan, employer or your medical team, among other health related topics. 

 

By accepting this Policy, you agree to release healthio from any and all liability and to hold healthio harmless for unauthorized use, disclosure, or access of your PHI sent to the email address or text number you provided.
